Compassion & Choices of New York provides information to people who are both terminally ill and to those who are simply planning ahead. We help educate people of the importance of having a health care proxy and will help people complete them. A form can be obtained by clicking here (pdf format) or by going to the New York State Department of Health website. We also provide copies of related documents, My Particular Wishes (pdf) and The Dementia Provision (pdf) developed by national Compassion & Choices. For those people who are unable to complete a health care proxy as there is no one available to be their health care agent, a Living Will Form, which can be used as evidence of one's wishes, can be obtained from Caring Connections.
